Medical Records Request

Information Request Release of Medocuments you will need and the purpose of request.
Request form should be signed/appoved by the patient’s Attending Physicians.
• During AP’s rounds – for admitted patients
• During the patient’s follow-up check-up
During request, please prepare one (1) valid Government ID to present to the Medical Records. In case the patient is not available, an authorization letter together with the valid ID’s of both patient and requestor/claimer should be presented.
A claim stub shall be issued for requests that will be processed for 3 to 4 working days.
• Clinical Abstract/ Discharge Summary shall take 3 to 4 working days.
• For admited patiens, the Request Forms are available at the Nurse Stations.
• For Walk-Ins, they may get an Authorization for Release of Medical Information Request Form directly from the Medical Records Window at the 2nd floor, Administrative Division Office.
Charge Slip shall be given on the day you will claim the requested documents.
Proceed to the Cashier (Located at the Ground floor) for payment.
Present the Official Receipt to the Medical Records Staff for documentation.
Certified True Copies of documents shall be released.
